&lt;a href="http://www.theblueform.com" rel="nofollow"&gt;www.theblueform.com&lt;/a&gt; They have a great drag and drop form builder as well as providing approval workflows which can be attached to the forms and mapped to existing business processes.  A lot more than just the usual online form builder here.</description>
